Causes

  • Dirty, clogged, leaking or mechanically worn fuel injector (cylinder 3 injector A).
  • Injector hydraulic characteristic is out of specification (flow variation).
  • Open, shorted, high resistance or intermittent injector wiring/connectors or poor ground to ECM.
  • Low or unstable fuel rail pressure or faulty fuel pressure regulator.
  • Engine mechanical problems causing cylinder contribution differences (low compression, valve issue).
  • Contaminated fuel (deposits affecting injector spray).

Symptoms

  • Malfunction Indicator Lamp (MIL) / Check Engine light on.
  • Rough idle or vibration, especially at idle.
  • Reduced performance, hesitation, stumbling or misfire felt under load.
  • Increased fuel consumption.
  • Diagnostic scan shows large learned injector offset/trim value for cylinder 3 or persistent offset-limit code.
  • Possible hard start or stumble on cold start.

What to check

  • Retrieve freeze frame and all stored codes; note any misfire (P03xx) or injector circuit (P020x) codes.
  • Use OEM-level scan tool to view learned injector offset/trim values and monitor live data for injector corrections.
  • Check fuel rail pressure at key-on and while cranking/running; compare to spec.
  • Measure injector resistance (ohms) and compare to specification.
  • Inspect injector connector and wiring for corrosion, damage, or poor pin fit; check for continuity and proper ground.
  • Perform cylinder contribution test or balance test to see if cylinder 3 is contributing properly.

Signal parameters

  • Injector pulse width (ms) / duty cycle while running.
  • Injector coil resistance (ohms).
  • Injector driver voltage/current waveform (V, mA).
  • Fuel rail pressure (psi/bar) at idle and under load.
  • PCM learned injector offset/trim value (counts or percent as shown by scan tool).
  • Battery voltage (V) and ECM ground integrity.

Diagnostic algorithm

  1. Capture freeze frame, DTC history and monitor live data. Verify P02D1 presence and any related codes (P0203, P0303, etc.).
  2. Review service bulletins and calibration updates for injector trim/learning for the specific vehicle.
  3. Visually inspect injector connector, harness and ECM grounds for damage, corrosion or looseness. Repair as needed.
  4. Measure injector resistance for cylinder 3 and compare to other injectors and specs.
  5. Check fuel rail pressure and regulator operation; verify adequate supply at idle and during cranking. Repair fuel system faults first.
  6. With engine running, monitor learned offset/trim and injector pulse widths. Use an oscilloscope to verify driver waveform and coil current if injector electrical fault suspected.
  7. Perform a cylinder contribution/balance test and a compression or leak-down test on cylinder 3 to rule out mechanical causes.
  8. If electrical checks are good and cylinder is mechanically healthy, swap the suspect injector with another cylinder. Re-scan: if the large offset follows the injector, replace the injector; if it remains on cylinder 3, suspect wiring/ECM or mechanical issue.
  9. Repair or replace faulty injector, wiring or PCM as identified. After repair, clear codes and perform required injector offset/trim relearn procedure with a proper scan tool per manufacturer instructions.
  10. Verify repair by rechecking learned offset values, running drive cycle and confirming MIL does not return.

Likely causes

  • Injector on cylinder 3 is partially clogged or leaking and requires cleaning or replacement.
  • Wiring harness connector at injector is corroded or has high resistance.
  • Low fuel rail pressure due to weak pump or leaking fuel pressure regulator.
  • Cylinder 3 has a mechanical condition (low compression or valve leakage) changing fuel requirements.
  • PCM has an internal fault or incorrect software/calibration preventing proper trim control.

Fault status

⚠️ Status
PCM reports learned injector offset for Cylinder 3 (Injector A) reached maximum allowed limit. ECM can no longer compensate for the imbalance; service required on injector, fuel system, wiring or cylinder mechanical condition. MIL illuminated.
