Causes

  • Vacuum or intake manifold leak on bank 2
  • Mass air flow (MAF) or MAP sensor fault/contamination
  • Faulty or slow oxygen sensor (O2) or wideband lambda sensor on bank 2
  • Fuel pressure low or unstable (fuel pump, regulator, filter)
  • Leaking/clogged/failed fuel injector(s) on bank 2
  • Exhaust leak upstream of O2 sensor or damaged catalytic converter

Symptoms

  • Check Engine MIL illuminated (P1178 present)
  • Rough idle or unstable idle speed
  • Reduced engine power, hesitation or stumbling under load
  • Poor fuel economy or black exhaust smoke (rich)
  • Failed emissions test or elevated tailpipe emissions
  • Occasional misfire or surging

What to check

  • Read and record all active/pending/frozen DTCs and freeze-frame data
  • Scan live data: short-term fuel trim (STFT), long-term fuel trim (LTFT), multiplicative factor/lambda for bank 2, O2/wideband sensor readings
  • Compare bank 1 vs bank 2 fuel trims and sensor behaviour
  • Visually inspect intake, vacuum lines, and PCV system for leaks or disconnected hoses
  • Inspect wiring and connectors for the bank-2 sensors (MAF, O2/lambda, injectors) for damage, corrosion or loose pins
  • Perform smoke test or pressure test of intake system to find leaks

Signal parameters

  • Long-term fuel trim (LTFT) typical range: about -10% to +10% (values outside ±20% indicate a problem)
  • Short-term fuel trim (STFT) typical range: about -25% to +25%
  • Multiplicative correction / lambda factor: normally near 1.0 (deviations >±10–20% are suspicious)
  • Narrowband O2 sensor voltage: ~0.1–0.9 V switching at idle (rapid switching indicates healthy closed-loop)
  • Wideband O2/Lambda: near 1.00 (stoichiometric); values 1.03 lean (vehicle-specific thresholds)
  • Fuel pressure: check against manufacturer spec (common rail/port systems often 3–4 bar for returnless systems, or higher for return-type)

Diagnostic algorithm

  1. Verify the code and capture freeze-frame and live data. Note whether problem is present at idle, cruise or under load.
  2. Check for related DTCs (O2 sensors, fuel trims, MAF/MAP) to narrow focus to sensors, air metering or fuel delivery.
  3. Visually inspect intake manifold, vacuum lines, PCV, and all hoses on bank 2. Repair any leaks, then clear codes and retest.
  4. Inspect wiring and connectors for bank-2 O2/lambda, injectors and air metering sensors. Repair or secure any damaged wiring or poor connections.
  5. Test fuel pressure and volume. If pressure is low or fluctuating, service fuel pump, filter or regulator and retest.
  6. Check MAF/MAP sensor readings for plausibility. Clean or replace MAF if contaminated; replace MAP if out of spec.
  7. Monitor O2/wideband sensor output for bank 2. If sensor is slow, stuck, or out of range, replace the sensor and re-evaluate fuel trims.
  8. If trims remain abnormal after sensors and fuel supply are verified, perform injector balance/flow testing and inspect exhaust for leaks or catalytic restriction.
  9. After repairs, clear codes and perform a full drive cycle to confirm the multiplicative correction returns to normal and code does not reappear.
  10. If problem persists and wiring and sensors are confirmed good, consider ECM software update or replacement as directed by manufacturer diagnostics.
