What to check

If the radio was recently replaced or updated, verify that the correct vehicle-specific software was applied. If not, obtain the correct software/calibration package.
Attempt a controlled software reflash/update of the radio using manufacturer-approved diagnostic tool and the correct files. Follow tool prompts exactly; do not interrupt.
After reflash, clear DTCs in all modules and perform a power-cycling sequence per manufacturer procedure. Re-scan for codes.
