Causes

  • Faulty or clogged fuel injector for cylinder 3
  • Ignition problem (bad spark plug, coil, or secondary wiring) on cylinder 3
  • Low compression or mechanical issue on cylinder 3 (valve, piston ring, head gasket)
  • Wiring or connector fault to injector or ignition driver
  • Intake manifold leak or external vacuum leak affecting cylinder 3
  • Fuel pressure or rail supply issue (intermittent)

Symptoms

  • Illuminated MIL (Check Engine Light)
  • Rough idle or vibration, especially at idle or light load
  • Reduced power or hesitation under acceleration
  • Poor fuel economy
  • Visible misfire count for cylinder 3 on scan tool

What to check

  • Retrieve freeze frame and live data with a scan tool; note misfire counts and cylinder contribution values
  • Check for related codes (ignition, fuel system, misfire, cam/crank correlation)
  • Inspect wiring and connectors for injector and ignition coil at cylinder 3
  • Swap coil or injector with another cylinder to see if the code/behavior follows the component
  • Perform a cylinder compression test and/or leak-down test on cylinder 3
  • Measure injector resistance and pulse width with a scope or scan tool; inspect injector for spray pattern if possible

Signal parameters

  • Injector pulse width (ms) — compare cylinder 3 to others; abnormal or much longer/shorter widths are suspicious
  • Short-term/long-term fuel trim (%) — look for deviations correlated to cylinder 3
  • Cylinder contribution/imbalance metric (%) or RPM variance reported by PCM
  • Misfire count for cylinder 3 (events/min or counts)
  • Compression pressure (psi or kPa) — compare cylinder 3 to spec and other cylinders
  • O2 sensor voltage or downstream HO2S trends — may show imbalance

Diagnostic algorithm

  1. Read and record all stored and pending DTCs and freeze-frame data. Verify MIL status and note conditions when code set (load, temp, rpm).
  2. Scan live data: monitor misfire counts, cylinder contribution values, fuel trims, injector pulse width for cylinder 3 and compare to other cylinders.
  3. Visually inspect ignition coil, spark plug, and secondary wiring for cylinder 3. Replace or swap coil/plug with a known good cylinder to check if problem follows the part.
  4. Inspect injector connector and wiring for corrosion, damage, or poor connection. Wiggle-test while monitoring live data for changes.
  5. Swap injector 3 with another cylinder's injector (or swap injector driver if accessible) and rerun contribution/misfire test. If imbalance moves, replace the faulty injector.
  6. Perform a compression test and/or leak-down test on cylinder 3 to detect mechanical issues (valve, piston rings, head gasket).
  7. Check fuel rail pressure under load and at idle to rule out supply issues; inspect for clogged fuel filter or regulator faults.
  8. If mechanical and fuel/ignition checks are normal, use an oscilloscope to inspect injector waveform, ignition primary waveform, and crank/cam signals for anomalies.
  9. Inspect intake manifold gasket, EGR/PCV systems, and vacuum lines near cylinder 3 for leaks affecting that cylinder.
  10. If all vehicle-level checks are normal, consider ECU/PCM driver faults and consult manufacturer service information before replacing control modules.
  11. Safety: take standard precautions when working on a running engine—avoid moving parts, hot surfaces, and high voltage from ignition components.

Likely causes

  • Clogged or failed fuel injector in cylinder 3
  • Faulty ignition component (coil or spark plug) on cylinder 3
  • Low compression in cylinder 3 (valve seating, ring wear, head gasket)

Fault status

⚠️ Status
MIL illuminated. PCM detected a cylinder 3 contribution/balance fault and stored code P0269 with possible freeze-frame data and misfire counts. Further live-data and component testing required to identify root cause.
