Causes

  • Faulty or contaminated upstream oxygen sensor (Bank 2)

What to check

  • Retrieve trouble codes and freeze-frame data with a capable scan tool
  • Record live data: bank 2 O2 sensor voltages (upstream and downstream) and switching behavior
  • Check short-term and long-term fuel trims for both banks (STFT/LTFT)
  • Inspect wiring and connectors for Bank 2 O2 sensor and injectors for damage, corrosion or shorting
  • Fuel pressure test at the rail with a gauge (compare to spec)
  • Visually inspect injectors for external leaks and listen for proper injector operation

Signal parameters

  • Bank 2 upstream O2 voltage: normally toggles ~0.1–0.9 V; a sustained >0.7–0.8 V indicates very rich
  • Bank 2 downstream O2: should be more stable but not constantly high (>0.8 V)
  • Short-term fuel trim (STFT) Bank 2: large negative values (e.g., < -10% to -25%) indicate commanded leaning to compensate for rich condition
  • Long-term fuel trim (LTFT) Bank 2: persistent negative LTFT (e.g., < -10%) supports rich condition diagnosis
  • Fuel rail pressure: compare to vehicle spec (commonly ~40–60 psi gasoline systems; refer to OEM spec)
  • MAF airflow (g/s) or voltage: compare to expected at idle / given RPM

Diagnostic algorithm

  1. Read and record DTCs, freeze-frame and live data (O2 voltages, STFT/LTFT, fuel pressure, MAF).
  2. Visually inspect wiring/connectors for Bank 2 O2 sensors and injectors; repair any damage. Check sensor harness pins and grounds.
  3. Compare Bank 1 vs Bank 2 O2 sensor behavior using live data. If Bank 1 is normal and Bank 2 is biased rich, focus on bank 2 circuit/components.
  4. Backprobe Bank 2 upstream O2 sensor: verify heater circuit and sensor signal at key engine conditions. Replace sensor if heater/signal out of spec.
  5. Perform fuel pressure test at idle and under load. If pressure is above spec, inspect fuel pressure regulator and fuel return. Repair or replace as needed.
  6. Inspect injectors on bank 2: perform injector balance/flow test or swap suspect injectors with bank 1 (if applicable) to see if fault follows injector. Replace leaking/stuck injectors.
  7. Clean or test MAF sensor and verify intake air values. Replace if faulty and consistent with rich symptoms.
  8. Check for fuel leaks (rail, injector O-rings) that could allow fuel dripping into intake or onto hot components.
  9. After repairs, clear codes and perform a road/drive cycle to verify code does not return and trims normalize.
  10. If no hardware fault found, check PCM software level and consult OEM service information/TSBs; as last resort consider PCM diagnostics or replacement.
