Causes

Diagnostic algorithm

  1. Record full freeze frame, live data and all stored codes. Do not clear codes yet.
  2. Visually inspect injector 3B connector and wiring for corrosion, damage or poor pin contact; repair any wiring issues.
  3. Check battery voltage and supply to injector circuit with key on and during cranking; verify proper ground.
  4. Measure injector coil resistance and compare with spec; note differences versus other injectors.
  5. Use a noid light or lab scope to verify injector drive signal (pulse width, shape, frequency). Look for clipping, missing pulses or unusual waveforms.
  6. Check fuel rail pressure and regulator operation. Repair fuel supply issues before replacing injectors.
  7. Perform a cylinder contribution/swap test: swap injector 3B with another cylinder and see if offset/code follows the injector or stays with cylinder 3. If the code follows the injector, replace the injector.
  8. If issue stays with cylinder 3 after swap, perform compression/leak-down and inspect intake/exhaust valves and injector seat for deposits or leaks.
  9. Clean or service injectors if contamination is suspected, then retest. In some cases ultrasonic cleaning or flow testing is required.
  10. After repair, clear codes, perform relearn/reset procedures per manufacturer (if required) and road test while monitoring fuel trims and injector learning values.
  11. If all components test within spec and the code returns, consider ECM software update or replacement and consult manufacturer technical bulletins.
  12. Safety note: take normal fuel-system precautions (no smoking, relieve fuel system pressure before disconnecting fuel lines).
